Home and Community


These services provide case management to individuals who are determined to be eligible for supportive home services as an alternative to institutionalization. Case Managers and nursing staff from State or contracted agencies work to determine eligibiilty and arrange for those services to be provided. They also monitor these services regularly to assure that high-quality care continues.

There are four such Home and Community Based Waiver programs.

The Elderly & Disabled Home and Community Based Waiver Program:

This home and community based waiver program is administered by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) . Services offered include all regularly covered Medicaid services (hospital, physician, lab, prescriptions) plus the following special waiver services:

The Assisted Living Waiver Program:

This home and community based waiver program is administered by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) . Services offered include all regularly covered Medicaid services (hospital, physician, lab, prescriptions) plus the services provided in a DSAAPD approved and appropriately licensed Delaware Assisted Living Facility.

The AIDS/HIV Home and Community Based Waiver Program:

This home and community based waiver program is administered by the Division of Social Services. Services offered include all regularly covered Medicaid services (hospital, physician, lab, prescriptions) plus the following special waiver services:

  • Case Management
  • Personal Care services
  • Respite Care
  • Supplemental Nutrition

Developmental Disabilities Home and Community Based Waiver Program:

This home and community based waiver program is administered by the Division of Developmental Disabilities Services (DDDS) . Services offered include all regularly covered Medicaid services (hospital, physician, lab, prescriptions) plus the following special waiver services:

  • Case Management
  • Day Habilitation
  • Institutional or In-home Respite Care
  • Residential Habilitation
