Greetings from the Delaware Public Health Office of Infectious Diseases and Epidemiology (OIDE). Thank you for taking the time to review the information presented on our webpage. It is our hope to join alongside other facilities statewide to promote the appropriate usage of antibiotics while combating multi-drug resistant organisms. Get Smart about Antibiotic Awareness Week
Get Smart Week is an annual observance to raise awareness of threats of antibiotic resistance and the importance of appropriate antibiotic prescribing and use. The next Get Smart Week is November 18th – 24th, 2023.
5 Ds of Antimicrobial Stewardship
• Diagnosis: Distinguish between true bacterial infections verse colonization, contaminations, or alternative diagnosis.
• Drug: Select the right drug considering guidelines, local antibiogram, formularies, drug-drug interaction, and allergy history.
• Dose: Calculate the right dose for the patients’ systemic function, weight, and age to minimize risk of toxicity.
• Duration: Choose the right duration of therapy to minimize risk of C. difficile infection, toxicity, or development of resistance.
• De-escalation: Narrow to an effective and safer lower risk agent based on culture data and other relevant parameters.
Reportable Healthcare Associated Infections
The following organisms are reportable in the State of Delaware:
• Candida Auris*
• Carbapenem Resistant Acinetobacter (CRAB)*
• Carbapenem Resistant Enterobacteriaceae (CRE)
• Carbapenemase-Producing CRE (CP-CRE)
• Clostridioides Difficile (C. Diff)*
• Extended Spectrum β-lactamases (ESBL)
• Methicillin-OR oxacillin- Resistant Staph Aureus (MRSA)
• Streptococcal diseases, invasive:
o Non – A/B
o Group A
o Group B
• Vancomycin Intermediate Staphylococcal Aureus (VISA)
• Vancomycin Resistant Enterococcus (VRE)
• Vancomycin Resistant Staphylococcus Aureus (VRSA)
*Not required by the State of Delaware (at the moment), but reportable to CDC NHSN
