For a copy of a survey report for a nursing home or assisted living facility, please go to the Survey Reports page on this Web Site. For other facilities please contact the Division’s New Castle County office. The Division does not issue other reports or prepare statistics on an ongoing basis. Please call the Division’s New Castle County office or Kent/Sussex Counties office if you need more information.
