How Do I Report Suspected Abuse, Neglect or Mistreatment?




The staff of DDDS take the mission to safeguard individuals served by the Division from any form of abuse, neglect, mistreatment, financial exploitation or significant injury seriously. If you suspect someone receiving services from the Division of Developmental Disabilities or any authorized provider is being abused, neglected or mistreated, please contact the Division.


To complete an online report of an incident of abuse, neglect, mistreatment, financial exploitation, or an individual rights complaint, use the DDDS Incident Reporting System.
