Sexually Transmitted Disease Reporting

All Delaware physicians, laboratories and other health care providers are required by regulations to report patients with sexually transmitted diseases to the Division of Public Health Sexually Transmitted Disease (STD) Program. Reporting enables appropriate public health follow-up for your patients, helps identify outbreaks, and provides a better understanding of disease trends in Delaware.

How to Report

  • Complete a Sexually Transmitted Disease Report Form ( PDF or Text ). Print only the PDF version, complete both pages and mail to the address on the form.
  • For rapidly reportable conditions, as indicated with a (T) in the list below, immediately contact the Division of Public Health STD Program.
  • Should you have any questions or concerns, please contact the STD Program Office.

Notifiable Diseases reported to the STD Program

  • Chancroid (S)
  • Chlamydia (S)
  • Gonorrhea (S)
  • Granuloma Inguinale (S)
  • Herpes (congenital) (S)
  • Herpes (genital) (S)
  • Human papillomavirus (genital warts) (S)
  • Pelvic Inflammatory Disease (N. gonorrhea, C. trachomatis or unspecified) (S)
  • Syphilis (S)
  • Syphilis (congenital) (T) (S)

(T) - report by rapid means (telephone, fax or other electronic means)
(S) - sexually transmitted disease, report required within 24 hours
