RADIATION TECHNICIAN FORMS AVAILABLE VIA INTERNET
DPH's Office of Radiation Control (ORC) has posted certification application, renewal and change of name and address forms for Delaware Radiation Technicians and Technologists at: www.dhss.delaware.gov/dhss/dph/regs.html#R. This new web content eliminates DPH's renewal mailings and provides greater accessibility for registrants.
Individuals already certified as Radiation Technicians or Technologists should submit their renewal forms approximately 30 days prior to the expiration date of their certificate. Certificate-holders should print their renewal form from the website, sign it, and mail with check or money order for $10 payable to the State of Delaware to: Division of Public Health, Office of Radiation Control, 417 Federal Street, Dover, DE 19901. Allow up to three weeks for processing. Renewed certificates will be mailed to certificate-holders.
Individuals who are not state-licensed practitioners must be certified by ORC to administer diagnostic or therapeutic procedures to patients at facilities located in Delaware. Certification is based on recognition of a national credential (ARRT, NMTCB, DANB or CCI), or successfully passing a Delaware State Limited Scope Medical or Dental Exam. Certification must be renewed every four years.
Technologists holding national credentials who apply for or renew a state certificate must include a photocopy of their national certificate or membership card in good standing with their application form, to the address specified on the form.
Certificate-holders who change their name must submit a change of name/address form, in order to obtain a certificate in their new name. Proof of name change, such as photocopy of marriage certificate or court papers, must be mailed to DPH. A certificate reflecting the new name will be mailed to the certificate-holder. Allow up to three weeks for processing.
X-ray machine facilities are now required to renew their registration on a periodic basis. Clients operating X-ray machines in their facilities are required to complete an application form to verify accuracy of information , modify as needed, sign and return approximately 30 days prior to the expiration date. The registrant will receive a new registration certificate to post in their facility. Allow up to three weeks for processing.
