Sample Delaware Relative Caregivers' School Authorization Affidavit

The Delaware Relative Caregivers' School Authorization Affidavit is required for a relative caregiver who is raising a child without legal custody or guardianship to register a child for public school.  Contact your local school district office to find out where the affidavit is being distributed within the district. School personnel will help you complete the affidavit.  You can download a sample affidavit here to help you prepare.
