Where can I get more information about advance directives?

You can read the Delaware law pertaining to advance health care directives.  It is part of the Delaware Code, which is available on line.  See Online Delaware Code  (Delaware Code Chapter 16, Sections 2501 to 2518).

You can contact the Division of Services for Aging and Adults with Physical Disabilities if you have questions about advance directives, if you need more information, or if you want a copy of the Advance Health Care Directive form.

Please remember that completing an Advance Health Care Directive involves making personal choices.  If you have questions or concerns about what decisions might be best for you, you might want to consider discussing them with family members, close friends, an attorney, your doctor, and/or a member of the clergy.
