A summary of the services provided by the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) and funded by the Division through the Older Americans Act, the social Services Block Grant, the Medicaid Waivers for Elderly and Disabled and Assisted Living and appropriations by the State of Delaware. Adult Day Services - provide activities and assistance for people with physical and/or mental challenges. Adult Foster Care - a housing option for people who need some supervision and can no longer remain in their own homes. Adult Life Skills - helps persons with disabilities who want to learn the skills to live more independently. Adult Protective Services - responds to cases of suspected abuse, neglect or exploitation of impaired adults. Alzheimer's Day Programs - day programs for persons with Alzheimer's, including medical, recreational and personal care services. Alzheimer's Respite - offers brief periods of relief to caregivers and gives them some time away from their caregiving responsibilities. Assisted Living - a residential option including personal services and light medical or nursing care. Assistive Devices - equipment that allows people to function more independently, ie, shower or bath chair, ramp, stair glide, etc. Attendant Services - provide personal care and support services so people can live in the community in their own home or with their family. Case Management - helps older persons, persons with disabilities and caregivers get connected to the services they need. Congregate Meals - provided daily in group settings such as senior centers. Medicare Senior Patrol - provides information to Medicare beneficiaries on how to detect possible fraud and abuse. Money Management – to promote and prolong independent living for persons who are at risk of losing their independence due to inability to manage their financial affairs. Emergency Response System – a device that allows a person to get immediate help in the event of an emergency. Hispanic Outreach – to help older persons in the Hispanic community learn about and get connected with the services they need. Home Delivered Meals – delivers nutritionally balanced hot meals during the day to homebound persons and cold, bagged meals for breakfast or dinner. Home Modifications – changes to a home to allow a person to move around more freely - wheelchair ramp, grab bars, wider doorways, etc. Housekeeping – an in-home service providing light cleaning to allow persons with disabilities and older persons who cannot do these tasks to remain in their homes. Information and Assistance – contact DSAAPD for information about the many services, programs, activities, events and other community resources related to aging and disabilities in Delaware. Joining Generations – a support/education group program for grandparents and relatives raising others’ children. Legal Services – provides help with matters such as power of attorney, living wills, consumer, housing and medical issues. Long Term Care Ombudsman Program – investigates and resolves complaints made by or on behalf of residents of long term care facilities. Medicaid Waiver Services – an alternative to nursing home care by providing needed services in the community setting. Medical Transportation – provides transportation to health care appointments for persons age 60 and over who live in New Castle County but outside of the City of Wilmington. Personal Care – assistance with personal hygiene, meal preparation, shopping, light housekeeping, etc. Respite Care – relief for primary caregivers. CARE Delaware’s goal is to support and assist caregivers so that they can continue in their caregiving roles. The information, assistance and resources available will help the caregiver to take the best possible care of their loved one and at the same time take care of their own health and well-being. Services include daily and short-term respite care, individual needs assessments and care planning and helpful information for caregivers. Highlights of some services are provided here. VieBridge Connections, CARE Delaware’s newest program is a membership website that provides caregivers with tools, resources and opportunities to interact with other caregivers within a virtual community. CARE Delaware’s social workers are trained professionals who can assist family caregivers who are struggling to care for a family member. They can assess the needs of the caregiver, assist caregivers in the development of a plan of care and provide information on services. The social workers not only help caregivers get connected with the services they need but also provide ongoing coordination and support. This service is available statewide. CARE Delaware supports 29 centers. Eight of the centers are staffed on a part time basis. The remaining 21 centers operate as libraries for videos, books, pamphlets and other materials on caregiving. Materials can be reviewed at the centers or checked out for home use. The staffed Caregiver Resource Centers, which are located in every county, are the community’s access point for information on a variety of caregiving issues. The coordinators at each center understand the challenges that caregivers face. They can provide the information, assistance and support that can be the most helpful to a caregiver’s individual situation. They will help caregivers navigate the array of services available, find solutions to individualized concerns and make appropriate choices. Many staffed centers also offer support groups. Christiana Care’s Evergreen Center I offers caregivers some time off from caring through a Saturday Adult Day Care program. The program accommodates frail elderly, 60 years of age or older, who are: 1) cognitively or mentally impaired; or 2) are unable to perform two activities of daily living (dressing, toileting, bathing, eating) without the assistance of another person. You do not have to have custody or guardianship of the child, but you must be related to him or her. KINSHIP CARE PROGRAM: The purpose of the Kinship Care Program is to assist caregivers during the 180 day transition period when a child initially moves into the non-parent caregiver’s home. MEDICAID: Medicaid is medical insurance that pays for doctors’ visits (including well- child visits), in-patient and out- patient hospital services, lab tests, behavioral health services, prescription medicines, eye exams, glasses, dental care for children, and over-the-counter medicines prescribed by your doctor. Eligibility for Medicaid is determined by income. Almost any child whose parent is not in the home and who has little or no income of his or her own can get Medicaid health insurance coverage.
