Behavioral Risk Factor Surveillance System(BRFSS) - Prevalence and Trends Data The BRFSS is the nation's premier system of health-related telephone surveys that collect state data about U.S. residents regarding their health-related risk behaviors, chronic health conditions, and use of preventive services.
CDC Community Health Status Indicators (CHSI) CHSI 2015 is an interactive web application that produces health profiles for all 3,143 counties in the United States. Each profile includes key indicators of health outcomes.

America's Health Rankings America’s Health Rankings releases two yearly reports, one on the health of the general population in the 50 US states plus a senior report on the population aged 65 and older in each state. Both reports include some health metrics that are stratified by race/ethnicity, gender, age, education, place of residence, and economic status.
County Health Rankings The County Health Rankings measure the health of nearly all counties in the nation and rank them within states. The Rankings are compiled using county-level measures from a variety of national and state data sources. These measures are standardized and combined using scientifically-informed weights.
