The Authorized Provider Committee collects information on several indicators of provider performance. Individuals who are in the process of selecting a day or residential service provider can use this information to help make their decision. Timeliness In Completing Essential Lifestyle Plans
This data details how each agency is doing in terms of completing Essential Lifestyle Plans (ELPs) within the 365 day timeframe for their Group Homes only. Number of Substianted PM46 Cases

Staff Turnover Rate

Service Terminations
This report indicates the number of persons the Service Provider has given notice to (e.g.: will no longer provide services to). Excluded are terminations resulting from changes in the person's health which require their move to a different type of setting outside of DDDS.
