Performance Indicators (Authorized Provider Compliance Reports)

The Authorized Provider Committee collects information on several indicators of provider performance. Individuals who are in the process of selecting a day or residential service provider can use this information to help make their decision. Choose the report you would like to view from the list below

Timeliness In Completing Essential Lifestyle Plans

This data details how each agency is doing in terms of completing Essential Lifestyle Plans (ELPs) within the 365 day timeframe for their Group Homes only. The data shows the total number of ELP's the agency has to complete in its Group Homes, as well as the percentage they completed on time (within the 365 days)

Number of Substianted PM46 Cases

Listed in this report is the number of substantiated PM46s for each Service Provider. The first column is the number of substantiated PM46s. The second column is the average number of persons served by that agency in the fiscal year indicated.

Staff Turnover Rate

This report includes information about the number of staff leaving each agency, the total number of staff, and the percentage of staff turnover.

Service Terminations

This report indicates the number of persons the Service Provider has given notice to (e.g.: will no longer provide services to). Excluded are terminations resulting from changes in the person's health which require their move to a different type of setting outside of DDDS.
